Elements Intake Online or Print
New to Elements Health? All new patients should complete the Elements Intake form above on their first visit to our clinic. If you have been to Elements Health previously, you do not need to complete this form.
Physiotherapy Intake Online or Print
Complete this form online if you are seeing Jaspreet for the first time. If you are seeing her for pelvic health, please only complete the form below.
Pelvic Health Intake Online or Print
Complete this form online if you are seeing Jaspreet for pelvic health physiotherapy. Please complete this form prior to your first visit.
Direct Billing
We direct bill for all eligible services. Please bring your insurance company information with you to your first visit. To save time, please complete your appropriate direct billing agreement(s) before you come. If you have Sunlife, please complete the Sunlife Agreement.
