At Elements Health, we are working to offer all of our paperwork online so you can complete it in the convenience of your own home. Completing our online forms works best on a computer or tablet.
Elements Intake
All new patients should complete the Elements Intake form above on their first visit to our clinic, along with the specific form(s) below for the service(s) you are seeking. If you have an appointment with our mental health team, your forms will be emailed to you separately.
Chiropractic Intake
Complete this form online if you are seeing a member of our Chiropractic team for the first time. Please complete this form prior to your first visit.
Chiropractic Child Intake (<10 years old)

Massage Intake
Complete this form online if you are seeing a member of our Massage Therapy team for the first time. Please complete this form prior to your first visit.
Physiotherapy Intake

Acupuncture Intake
We have switched to the Jane system for booking acupuncture. Please log into your Jane account (check your email for the link to set up your account) or complete the forms at your first visit.
Direct Billing
We direct bill for all eligible services. Please bring your insurance company information with you to your first visit. To save time, please complete your appropriate direct billing agreement(s) before you come. For Sunlife please complete the Sunlife Agreement form below. For all other benefit plans, please complete the Elements Health Agreement. Not sure which forms to complete, contact us anytime!
